COVID-19 information for patients and families

To help protect our people, patients and communities, we are screening everyone who enters our main hospital buildings. Some of our entrances may be closed or have reduced access.

All clinics will be closed on Monday, June 8 for a planned closure.

We will have an Urgent Care Clinic at St. Jamestown Health Centre (410 Sherbourne St, Ground Floor)
open from 9 a.m. - noon and 1 - 4 p.m.

All patients will require phone pre-screening: Phones open 9 a.m. to 4 p.m.

Important information about changes to locations for Family Health Team appointments

To all of our patients, as of Monday, April 20, we are combining our clinics into two locations:

Health Centre at 80 Bond

  • Patients of the 61 Queen St. East Family Practice and St. Lawrence clinics will have their appointments at 80 Bond. St. Lawrence will remain open for in person appointments on Thursdays.
  • Patients of the 80 Bond clinic will continue to have their appointments at 80 Bond.

Sumac Creek Health Centre

  • Patients of the Health Centre at 410 and St. James Town clinics will have their appointments at Sumac Creek. 410 and St. James Town will be open for in person appointments on Wednesdays.
  • Patients of Sumac Creek clinic will continue to have their appointments at Sumac Creek.

Please review the table below to see where your in person appointment will be. Virtual appointments by phone or video will still be offered outside of these hours depending on your doctors' availability.

If you have health concerns outside of these hours on the weekend please use our after-hours phone line.
After-Hours Urgent Care (ask for the family medicine resident on-call)

Please call your clinic number to be connected with our weekend and after hours urgent care.

Call ahead to book an appointment

We are here for you during these challenging times if you need medical care during regular business hours, on the weekend, or after-hours you can call your clinic to speak with a member of our team.

If you are calling after 5 p.m., and have an urgent medical issue that is not a medical emergency, you can contact the St. Michael’s locating department and ask to speak with the family medicine resident doctor on call. You can reach locating by calling your clinic, and pressing “1” to be connected.

If you are asked by one of our team members to come to a clinic, you will be given directions, an appointment time and will be screened at the door before entering the clinic.

If you must come to 80 Bond St. or Sumac Creek without phoning first, you will be screened at the front door and will be asked about your health issues. Instead of being seen in the clinic, an appointment may instead be booked for a telephone visit at a later date.

Appointment information - including telephone and video options

In order to prevent the spread of COVID-19, starting immediately, we will be rescheduling all non-urgent in-person patient appointments until further notice. To continue to provide the best possible care to our patients, we will be offering virtual care visits such as telephone appointments and video visits (eVisits) to patients to address their health concerns.

Family Health Team (FHT) visitor guideline

Due to increasing COVID-19 community transmission and recommendations from the Ministry of Health we are limiting visitors at the FHT.  If you are a patient and are coming in to the clinic, we are only allowing people described below to come with the patient:

  • A caregiver
  • A support worker
  • One parent/guardian for patients under the age of 16
  •  A support person helping with language barrier

All other people will not be allowed to come with the patient to their clinic visit.

Thank you for your understanding in this matter.  This decision was made to slow the spread of the virus and follow social distancing practices.
